Diabetes Log

Ive been to loads of trouble finding one that isn't so confused and seems okay. I have problems with remembering to write it so I think it would be better to have it on my computer so that patients see and not forget. Help? By the way I work on Windows, Mac or not

Me personally I just use a small note book
I keep right with my meter
with a small kitchen timer
remember to take a test at the
appropriate times of the day
